Six providers. Two locations in New York City. And a vaccine program that was costing more than it was bringing in.

Premier Pediatrics was a thriving practice with a vaccine program that had quietly become a financial drain. This is the story of how they turned it around.

Independently Owned 6 Providers 2 Locations 98% Private Insurance Manhattan & Brooklyn

About Premier Pediatrics

Dr. Julie Capiola runs Premier Pediatrics out of two locations in Manhattan and Brooklyn. It is a real multi-provider operation: six providers, independently owned, serving families across two of the most densely populated boroughs in the country. The practice administers more than 10,000 vaccines every year.

This is not a small practice struggling to get by. Premier Pediatrics is a successful, well-run organization with a strong patient base and a reputation for quality care. The practice was growing. The providers were busy. The families kept coming back.

The problem was specific: the vaccine program had become a drain. Nearly all of their patients, 98%, carry private insurance. That means every single vaccine involves purchasing inventory, documenting administration, submitting claims, and chasing reimbursement. At 10,000+ doses a year across two locations, the sheer volume of that work had created a cost structure that was quietly eating the practice from the inside.

The Challenge

What They Were Dealing With

The numbers looked alarming when you added them all up. Premier Pediatrics was carrying over $1 million a year in vaccine-related costs. That includes the cost of purchasing vaccine inventory, the billing and admin fees on every claim, the overhead of managing documentation across two sites, and the waste that comes with maintaining stock at the scale they were operating.

The billing fees and administrative overhead on each vaccine were higher than the margins the practice was making on those same vaccines. That is the math that catches a lot of practices off guard: you can be administering thousands of vaccines and still be losing money on the program as a whole, once you account for everything it takes to bill, track, and manage the process.

And then there was the staff problem. Vaccine documentation was eating hours of clinical time every single day. At Premier Pediatrics, every vaccine administered required manual documentation that took upwards of 10 minutes per dose. When you are giving 10,000+ vaccines a year, that is over 1,600 hours of staff time just on documentation. The workload was burning people out. Staff turnover was high, and every departure meant hiring and training someone new, which added more cost and more disruption.

On top of all of that, vaccine waste was adding insult to injury. Multi-dose vials that expired before they could be used, inventory that sat too long in one location while the other ran short. A program that was supposed to generate revenue had become a net negative when you tallied up all the hidden costs.

The Solution

What Changed

Canid replaced the entire cost structure around Premier Pediatrics' vaccine program. Here is what that looked like in practice.

The first thing that changed was the most expensive part: Canid took over vaccine purchasing entirely. Premier Pediatrics stopped buying vaccines. No upfront costs, no inventory risk, no capital tied up in refrigerators full of product that may or may not get used before it expires. That alone removed the single largest line item from their vaccine cost structure.

The second change was the one that the staff noticed immediately. Documentation that used to take 10 or more minutes per vaccine now took 30 seconds. One barcode scan at the point of care, and the entire record was captured. No more manual entry of lot numbers, expiration dates, administration sites, or payer details. The clinical team could actually focus on patients again instead of spending their days hunched over paperwork.

With 98% of the patient population on private insurance, Premier Pediatrics was dealing with a long list of payers, each with their own billing quirks, modifier requirements, and reimbursement timelines. Canid's dedicated billing team took all of that off the practice's plate. Claims, follow-ups, denials, appeals. The practice stopped thinking about vaccine billing entirely.

And both locations, Manhattan and Brooklyn, were integrated into one system. Instead of managing two separate workflows, two separate inventories, and two separate billing streams, the practice had a single view across everything. One partner handling it all.
